Company Description

The Credit Valley Family Health Team (CVFHT) is a vibrant academic FHT affiliated with The Credit Valley Hospital and the University of Toronto.  It is composed of 9 Family Physicians, nurses, a nurse practitioner, social workers, a pharmacist, dietitians, a chiropodist and two diabetes teams.  We have two sites -  Academic Site A and Bilingual Site B. The CVFHT’s Mission of Excellence is to provide high standard interprofessional primary care and family medicine teaching.
