The Registered Nurse, Case Manager leads the surgical and non-surgical team and is responsible for the effective coordination of the pre and post surgical management of the patients who are having a hip or knee joint replacement. The Case Manager works closely in a multi-disciplinary team which includes the patient, family, Surgeon, Physiotherapist, Occupational Therapist, Dietitian and external stakeholders. The Case Manager evaluates, plans and implements the plan of care for the patient in this outpatient orthopedic clinic. As the focal point through which patients access a range of services, the Case Manager collaborates with patients on behalf of the surgeon, to ensure they are optimized for surgery. The role of the Case Manager includes; working in a clinic setting with the Surgeon; completing pre-operative teaching with patients and families; ensure patients are optimized for surgery; preparing the surgical chart for the hospital and guiding patients through their post-operative care needs once they are discharged from hospital. Candidates will be a Registered Nurse in good standing, Orthopedic or Pre-Admission clinic experince is an asset. Hours of work are Monday to Friday. Flexability in work hours will be required due to the nature of the work. There is one (1) casual position available to start with the potential of it becoming temporary full time.
