Healthy Relationships, and Hospitable Environments.  Reporting to the Regional Community Manager, you will be responsible for coordinating the delivery of care for The Good Samaritan Society CHOICE programs.

Job Description

The Good Samaritan Society CHOICE Program (Comprehensive Home Option for Integrated Care of the Elderly) provides seniors with complex medical conditions the advantage of living at home by utilizing the support services made available through our community based program.  You will be responsible for leading and supporting the multi-disciplinary teams that deliver client care.  The CHOICE teams are comprised of physicians, social workers, a chaplain, nurses, rehabilitation therapists, a dietician and other health care workers.  Our clients come to the program to access a broad range of services including primary care and rehabilitation services.

Through participative management you will lead, inspire and coach employees to excellence.  You will be responsible for:
  • Fostering an environment that is client-centered with a strong focus on client and employee satisfaction
  • Supporting quality improvement and best practice initiatives while acting as the clinical lead CHOICE programs
  • Ensuring care and clinical services are in compliance with GSS policies and AHS standards and legislation.
You are a Registered Nurse with active CARNA registration and minimum 2 years management experience. A BScN is required and previous experience working in geriatrics is preferred.  You have experience working in community with a collaborative and flexible community based approach.  Dementia care training would be an asset.  You demonstrate strong communication, organization and interpersonal skills.  Strong computer skills are essential.  Must have a valid driver's license and have access to a vehicle for travel between programs.
All new employees are required to submit:
- a current and clear Criminal Record Check and if applicable, a Vulnerable Sector Check that has been obtained no more than 90 calendar days prior to the hire date;
- a tuberculosis screening that has been completed no more than 12 months prior to the hire date.
