Job Description

POSITIONS: Addictions Counsellor, Community Withdrawal Management, 1.0FTE, and Opioid Outreach and Treatment, 1.0FTE


The Opioid Outreach and Treatment Services Program provides comprehensive assessment and treatment services to those affected by opioid use.  The service is based on a philosophy of harm reduction that maximizes client choice and participation in goal setting.  The program brings together a team of nurses, case managers and peer workers to assist and support individuals and their families/significant others.  A majority of program services are offered in the community through outreach support.  

The Community Withdrawal Management Program provides clinical services to clients seeking withdrawal management support.  Services will include withdrawal planning, symptom monitoring and counselling support, comprehensive clinical assessment and treatment planning and transitional short term case management.  Services will be delivered throughout the Halton Region

Essential Duties and Responsibilities in the Opioid Program

  • Provide specialized opioid use and nursing assessments services through a combination of diagnostic tools, individual structured interviews and motivational interviewing
  • Develop a comprehensive treatment plan in partnership with the client based on a thorough review of assessment information.
  • Facilitate the implementation of the treatment plan including provision of individual counselling, support, coordination and monitoring of appropriate referrals to additional specialized services and ongoing review and development of the treatment plan.
  • Provide education and support on critical issues of harm reduction, OST and risk management practices for opioid use.
  • Coach clients on issues related to medication, withdrawal management and other health related issues
  • Liaise with physicians and pharmacists as appropriate to support client care and treatment goals
  • Assist with program development and changes related to best practices in nursing.
  • Provide nursing support and education to other ADAPT staff /programs as required
  • Collaborates with community partners regarding client care, opioid education and case management
  • Participates in community education and program awareness initiatives
  • Maintain accurate computer data entry as per agency standards
  • Maintain accurate and complete client file documentation as per agency standards.

Essential Duties and Responsibilities in the Community Withdrawal Management program:

Direct Clinical Services – Withdrawal Management Counselling

  • co-ordinates and participates in the direct clinical care of individuals seeking withdrawal management services
  • conducts initial withdrawal symptom screenings and comprehensive withdrawal management assessment and treatment planning
  • in consultation with the client and any additional health care providers (as appropriate) develops a plan for safe withdrawal
  • regularly monitors and assesses client withdrawal symptoms
  • provides office based, community based and outreach support for clients within the Halton Region
  • provides individual withdrawal management counselling to address the Clients’ needs

Direct Clinical Services – Transitional Case Management:

  • assesses client needs through the use of diagnostic tools (including but not limited to GAIN and OCAN) and structured interviews
  • in consultation with the client, develops an individual Treatment Plan for the client
  • facilitates referrals to appropriate support services and agencies according to the clients’ need
  • provides transitional short term addiction or concurrent disorders transitional case management support 



  • Current certification with the College of Nurses of Ontario as a Registered Nurse or as a Registered Practical Nurse;
  • Current personal liability insurance protection with RNAO/RPNAO or equivalent provider;
  • Minimum 3-5 years of nursing experience;
  • Strong interpersonal and communication skills;
  • Some experience with basic CBT counselling skills an asset;
  • Experience in individual, family and group work;
  • Concurrent disorder and trauma informed knowledge/skills are a valued asset;
  • Basic working  knowledge of opioids and opioid withdrawal;
  • Ability to work independently and to function as a member of a team;
  • Comfort and experience with Harm Reduction as a treatment approach/philosophy;
  • Knowledge of and comfort with the outreach approach to treatment;
  • GAINS Certification an asset;
  • Prior training and experience using CATALYST client data system an asset;
  • Ability to use common office software and to adapt to new software and applications;
  • Must have a valid driver’s license and access to a reliable vehicle;
  • Must undergo a police records check;
